Solaris 9 release upgrade failed

What we need to do is upgrade the current Solaris 9 release 8/03 to
the latest which is 9/04. When I run the upgrade install and when it
runs through and does all its things, it comes back and give me a
"failed" with the following details:

Checking c1t0d0s0 for an upgradeable Solaris image
Checking c1t2d0s0 for an upgradeable Solaris image
Unable to read clustertoc
Checking c3t0d0s0 for an upgradeable Solaris image

"... all the way down to..."

Upgrading mirrored root on d0 (c1t0d0s0 c1t1d0s0) in place of c1t0d0s0
Error: Mount failed for either root, swap, or other file system

Solaris Volume Manager is mirroring the volumes. I think it failed
because of the mirror and I would need to remove the mirror? Has
someone came across this?
