SUMMARY: HBA (QLA2100) Drives not Visible After Solaris 8 Upgrade

The resolution was to copy the extra entries in the sd.conf file I had
running under 2.7 to /kernel/drv/sd.conf. The devices had SCSI ID's >
15 which were not defined in the sd.conf that the 2.8 upgrade had
written.
After replacing the extra sd.conf entries, I rebooted. It came up
cleanly, and metainit didn't complain as it had been doing in the past.
I picked up the additional metadb replicas on the external drives, so I
didn't need to make any adjustments to the metadb's. I have mounted the
filesystem read-only and it looks like all of the data is intact.

I have upgraded an Ultra-80 from Solaris 2.7 to 2.8.
The system partitions are on the internal drive (c0t0d0) and an external
array is attached with a QLogic QLA2100 copper fiber channel HBA. The
system drive and various tape and cdrom devices are visible and
functioning well under 2.8. The devices c1t34d0s?, etc are not visible
to the system after upgrading to Solaris 2.8. I have updated the
qla2100 driver from the 2.x version that run under Solaris 2.7 to the
most recent version 3.0.3--which is designed to work with Solaris
2.8--and the results are the same.
The QLA2100 driver loads and the yellow warning light on the Polaris
drive enclosure goes off, indicating the HBA's presence. It appears
that the HBA recognizes the drives, as shown by dmesg:

Has anyone performed a Solaris 2.7 -> 2.8 upgrade with one of these
adapters or installed on from scratch on a 2.8 system? If so, could you
send me the "recipe" you followed? Any suggestions are welcome.
FURTHER DETAILS:
The 9 drives in the array were concatenated into one metadevice (md0)
under Solaris 2.7.
Following the steps outlined in a previous posting "SUMMARY: Preserving
DiskSuite metadevices across and upgrade", I have come to a wall at step
5, where I am to run metadb -a -f deviceA deviceB ...
The metadb -a -f command fails with "No such device or address", as follows:
