locking a user

I managed to implement a pam module in pam.conf to log a user out after 3 unsuccesful logins.
I cannot however login in even though the account is unlocked.
I have tried resetting the password and tried passwd -d username.
The /etc/shadow files shows the account as unlocked.
What am I doing wrong?
